Qualifications for Eligible Applicants

Immediate family has lost a parent(s) or guardian(s) within the last year*

Immediate family has at least one child under the age of 21

All family members within the application are U.S. citizens

Please include the following information in your application form:

  • Nominated Family's email address, phone number, and mailing address.

  • Letter sharing your situation including family details and reason for nomination.

  • Proof of situation (ex: obituary)

* Immediate family includes remaining parent/guardian and their children. Any special situations will be determined by our board.
